Omicron is serious. "Very interesting analyses about the virology of #Omicron, which may explain the faster spread of this variant.
According to a new lab study, Omicron infects & multiplies ~70x faster than the Delta variant and the wild type SARS-CoV-2 in the human bronchus, but not in the lung.
In this ex vivo study (press release), Michael Chan, Malik Peiris & John Nicholls et al. @hkumed show that at 24h after infection Omicron replicated ~70x faster than Delta in bronchus. Interestingly, it replicated ~10x less efficiently in the lung tissue. hkumed.hk/96b127/ Image

Another analysis by @BalazsLab also supports these findings. In this lab study w/ pseudoviruses, Omicron showed greater ability to infect cells than other variants, which was ~ 4 times more infectious than the original strain, also more than Delta. medrxiv.org/content/10.110…
These preliminary laboratory analyses indicate that;
📍Omicron is significantly more transmissible than delta
📍Less efficient replication in the lungs may suggest lower severity, but severity in humans is not determined only by virus replication but also the host immune response
Important to note that first study is an ex vivo work, meaning that human tissue is used to look at virus replication in the lab. So, this is not a definitive but supportive finding & needs to be confirmed by clinical data as host immune response is absent in these studies.
Lay summary: These results indicate inherent transmissibility & replication advantage of Omicron (in addition to immune evasion) compared to delta. It also explains why it spreads quickly and outcompeted delta in SA, which fits w/ modelling assumptions.

The only thing I am sure of is that omicron will spread so quickly through the population, making it likely impossible to contain even with the most stringent measures & giving us very little time over the next few weeks. So get your vaccines & boosters! "

Commentary: Faster replication in the nose and throat also means much faster spread. The bottom line is that vaccines alone aren't the guarantee they were 3 months ago. Everyone needs to be masked up with the best masks available to us, N95 or better.
If you're planning on traveling for the holidays, make sure you and everyone you'll be visiting is vaccinated and boosted to the maximum permissible amount. If you or someone you might be visiting is immune-compromised or otherwise at risk, you may wish to reconsider or at the very least, make sure you have rapid tests on hand. The faster replication also means tests should detect contagiousness sooner.

If you have a choice, get Moderna for vaccine/boosters. "The recently emerged SARS-CoV-2 Omicron variant harbors 37 amino acid substitutions in the spike (S) protein, 15 of which are in the receptor-binding domain (RBD), thereby raising concerns about the effectiveness of available vaccines and antibody therapeutics. Here, we show that the Omicron RBD binds to human ACE2 with enhanced affinity relative to the Wuhan-Hu-1 RBD and acquires binding to mouse ACE2. Severe reductions of plasma neutralizing activity were observed against Omicron compared to the ancestral pseudovirus for vaccinated and convalescent individuals. Most (26 out of 29) receptor-binding motif (RBM)-directed monoclonal antibodies (mAbs) lost in vitro neutralizing activity against Omicron, with only three mAbs, including the ACE2-mimicking S2K146 mAb, retaining unaltered potency. Furthermore, a fraction of broadly neutralizing sarbecovirus mAbs recognizing antigenic sites outside the RBM, including sotrovimab, S2X259 and S2H97, neutralized Omicron. The magnitude of Omicron-mediated immune evasion and the acquisition of binding to mouse ACE2 mark a major SARS-CoV-2 mutational shift. Broadly neutralizing sarbecovirus mAbs recognizing epitopes conserved among SARS-CoV-2 variants and other sarbecoviruses may prove key to controlling the ongoing pandemic and future zoonotic spillovers."

Commentary: What the associated study shows is that Moderna's vaccine and boosters respond best against Omicron, followed by Pfizer. Some vaccines like Sputnik, J&J, and AstraZeneca have lost some or all effectiveness against Omicron. If you have a choice, get Moderna for your vaccination or booster. That said, any booster is better than none, just as any vaccine is better than none.

A reminder of the simple daily habits we should all be taking.
1. Wear the best mask available to you when you'll be around people you don't live with, even after you've been vaccinated. Respirators are back in stock at online retailers, too. Wear an N95/FFP2/KN95 that's NIOSH-approved or better mask if you can obtain it. If you can't get an N95 mask, wear a surgical mask with a cloth mask over it.

3. Get vaccinated as soon as you're able to, and fulfill the full vaccine regimen, including boosters. Remember that you are not vaccinated until everyone you live with is vaccinated. If you received an adenovirus vaccine (J&J/AstraZeneca), consider getting an mRNA single shot booster (Pfizer/Moderna) if available.
4. Wash/sanitize your hands every time you are in or out of your home.
5. Stay out of indoor spaces that aren't your home and away from people you don't live with as much as practical. Minimize your contact with others and avoid indoor places as much as you can; indoor spaces spread the disease through aerosols and distance is less effective at mitigating your risks.
6. Aim to have 3-6 months of living expenses on hand in case the pandemic gives another crazy plot twist to the economy.
7. Replenish your supplies as you use them. Avoid reducing your stores to pre-pandemic levels in case an outbreak causes unexpected supply chain disruptions.
8. Ventilate your home as frequently as weather and circumstances permit, except when you share close airspaces with other residences (like a window less than a meter away from a neighboring window).
